New Patient Registration
You can register with the practice by either:
- coming into the surgery and telling the receptionist you would like to register as a new patient. Please bring with you your NHS number which will help us to register you quickly.
- completing the attached registration form and sending it to the surgery. Please ensure that all sections are completed and that it contains your NHS number.
It may take a little time for your records to arrive from your previous doctors. We will therefore ask you to fill in a short form so that we have some background information about yourself and a note of any major problems you may have. You will also be offered an appointment with one of the doctors so that he may go over this with you.
