New Patient Registration

You can register with the practice by either:

  • coming into the surgery and telling the receptionist you would like to register as a new patient. Please bring with you your NHS number which will help us to register you quickly.
  • completing the attached registration form and sending it to the surgery. Please ensure that all sections are completed and that it contains your NHS number.

It may take a little time for your records to arrive from your previous doctors. We will therefore ask you to fill in a short form so that we have some background information about yourself and a note of any major problems you may have. You will also be offered an appointment with one of the doctors so that he may go over this with you.

Medical treatment is available from the date of registration. Please contact reception for further information.
