If you need to update your mobile number or email address, please speak to Reception.
If you are updating your name or address, you will need to provide proof. This could be a letter, passport, marriage certificate, bank statement etc.
If you are in the surgery, you will be asked to complete a form with your updated details and the Receptionist will sign to say they have seen ID.
Alternately if you are unable to come into the surgery, you can send in the proof of change via a photo.
- Email your updated details and proof to: reception.northhouse@nhs.net
- Ask Reception to send a link to your mobile for you to attach a photo.
